
(September 25, 2025; Monrovia, Liberia): NDMA’s Executive Director Hon. Atty. Ansu V. S. Dulleh Sr., on yesterday chaired a Special Panel Investigative Committee of Inquiry comprising the National Disaster Management Agency (NDMA), Liberia National Fire Service (LNFS), Liberia National Police (LNP) and the Environmental Protection Agency (EPA), to finalise findings on the cause(s) of the oxygen tank explosion which took place at the MIRA Gas Company on the Bushrod Island, leading to the death of two persons and several sustaining serious injuries.
As part of ongoing investigation into the oxygen tank explosion, MIRA Gas Company was invited by the Committee of Inquiry to find out and make determination on the following:
1. Safety Assurance – Oxygen is highly reactive, and any explosion of such poses severe risks to life and property. The Investigation will help identify the exact cause(s) to prevent recurrence.
2. Prevent Future Accidents – Findings out of this inquiry will guide improvements in manufacturing, storage, handling, and usage procedures to reduce future risks.
3. Accountability – The investigation will further establish whether the explosion was due to human error, equipment failure, poor maintenance, or non-compliance with safety standards.
4. Public Confidence – The investigation is also expected to reassure the public and the workers that safety is prioritized and corrective actions are being taken.
5. Legal and Regulatory Compliance – Explosion of such may involve liability issues, insurance claims, or violations of safety regulations. The investigation will ensure that proper legal steps are taken.
6. Technical Knowledge – The investigation will finally add to understanding how oxygen systems behave under different conditions, contributing to better engineering and safety standards, so that the Government of Liberia will make a determination on the next course of operational activities of the company.
MIRA Gas Company was represented by Nathaniel K. Zoegar, the company’s Health and Environmental Safety Consultant and Sallay T. Randolph, Human Resource Services Consultant of MIRA Gas Company.
Findings of this investigation and recommendations to the Government of Liberia which are expected to come out in the next few days, will be made known to the public.
